USER SERVICES Durable Medical Equipment: User Intake Packages Purpose After developing policies that specify services offered, eligibility requirements, fees (if any), and the terms of service or use, the AT organization will need to develop a set of documents to implement those policies. Several of those documents will comprise an application and/or intake package. Examples The attached documents are examples of user intake forms for durable medical equipment (DME)from different reuse programs across the country. Also attached are examples of procedures.
