USER SERVICES Assigned Device: Delivery and Liability Release If a user is matched with an assistive technology device at a center, basic instruction should be given in the use of the device. This is both a service to the user and a means of mitigating risk associated with providing a used device. The organization should develop appropriate training or instruction modules for each device offered. The staff member providing the instruction should be thoroughly familiar with the approved instruction module. This staff member may be the person who delivers equipment to the home, if delivery service is provided. After the user has received instruction in the use of the device, he/she should be asked to sign a document acknowledging that the instruction was received. This document should be retained in the user's record. See the attached example. The device reipient also may be asked to sign a release from liability. An example is attached.
